Why CBD Affects Blood Thinner Levels
The liver uses enzymes (primarily CYP2C9 and CYP3A4) to break down warfarin. CBD inhibits both of these enzymes. When those enzymes are slowed down, warfarin isn't cleared from the body as quickly, and its concentration in the blood rises.
Higher warfarin levels mean a stronger blood-thinning effect than intended. This increases the risk of bleeding, including dangerous internal bleeding. The degree of this interaction depends on the CBD dose, your individual liver metabolism, and your warfarin dose.
What the Research Shows
Multiple case reports have documented elevated INR (International Normalized Ratio, the measure of blood clotting) in patients who added CBD to a warfarin regimen. In these cases, warfarin doses needed to be reduced to keep INR in a safe range.
A pharmacokinetic study found that CBD can increase warfarin exposure (area under the curve) significantly. This isn't a theoretical concern. It's a documented, real-world drug interaction.
Signs Your Warfarin Level May Be Too High
If you start CBD while on warfarin and notice any of these symptoms, contact your doctor promptly:
- Unusual or excessive bruising
- Bleeding from the gums when brushing your teeth
- Blood in urine (pink or red color)
- Blood in stool (black, tarry stools or bright red)

What About Newer Blood Thinners?
Newer anticoagulants (NOACs or DOACs), including apixaban (Eliquis), rivaroxaban (Xarelto), dabigatran (Pradaxa), and edoxaban (Savaysa), have different metabolic pathways than warfarin. Here's what's known:
- Apixaban (Eliquis): Metabolized partly by CYP3A4, which CBD inhibits. The interaction may be meaningful. Monitor for unusual bruising or bleeding.
- Rivaroxaban (Xarelto): Also CYP3A4-dependent. Similar concern as apixaban.
- Dabigatran (Pradaxa): Not CYP-metabolized. The interaction with CBD is less clear, but dabigatran uses P-glycoprotein pathways that CBD may also affect.
Bottom line: all blood thinners carry some level of interaction concern with CBD. None of them should be combined with CBD without telling your prescribing doctor.
If You Take a Blood Thinner and Want to Try CBD
Here's a practical approach to discuss with your doctor:
- Tell your doctor you're considering CBD before you start. Don't surprise them with it at your next appointment after already starting.
- Start with a very low CBD dose if your doctor agrees it's okay. A 5mg dose causes less enzyme inhibition than a 25mg dose.
- Get your INR checked more frequently in the first few weeks. For warfarin users, this may mean moving from monthly to weekly checks initially.
- Watch for bleeding symptoms as described above and report any changes immediately.
- Don't adjust your warfarin dose yourself. If your INR changes, your doctor should adjust warfarin, not you.
