How to Talk to Your Doctor About CBD and Medicare

Talking to your doctor about CBD doesn't have to be awkward. Most doctors today are at least familiar with CBD, and many are genuinely open to the conversation. The key is being direct and specific. You're not asking for a prescription. You're asking whether your doctor's ACO participates in the Substance Access Benefit Enhancement Initiative (BEI), which lets eligible Medicare patients receive qualifying CBD at no cost.

For context on how the BEI works, see our Medicare CBD overview.

Quick Facts
  • You don't need a prescription to get CBD through the BEI, but your doctor must enroll you
  • Many doctors aren't yet familiar with the BEI and need the program name to look it up
  • Being specific helps: ask about the "Substance Access BEI" by name
  • If your doctor is open to CBD generally, they're likely open to exploring the BEI
  • Mentioning your specific concerns (sleep, pain, inflammation) frames the conversation productively

Why This Conversation Feels Hard

Many people worry their doctor will be dismissive or think they're asking for something recreational. That hesitation is understandable, but it's largely outdated. Hemp CBD is legal, widely used, and increasingly part of mainstream wellness conversations. Your doctor has almost certainly been asked about it before.

The real challenge isn't judgment. It's that many doctors simply don't know about the BEI program yet. That means your conversation might go better if you come prepared with the program's name and a simple explanation of how it works.

What to Say: Scripts You Can Use

Opening the Conversation Simply

"I've been reading about CBD for [sleep / joint pain / inflammation]. I wanted to ask if that's something you think could work for me, and whether your practice participates in any Medicare programs that cover it."

Asking About the BEI Specifically

"I read that some Medicare ACOs participate in a CMS program called the Substance Access Benefit Enhancement Initiative that provides CBD products to patients at no cost. Does your ACO participate in that?"

If They're Unfamiliar With the BEI

"It's run through the CMS Innovation Center as part of the ACO REACH model. Your ACO coordinator might know more about it. I wanted to ask in case it's available to me."

What to Expect

A few possible outcomes:

  • Your doctor's ACO is enrolled and they can walk you through getting enrolled in the benefit. Great outcome.
  • Your doctor doesn't know about the BEI but is willing to check. They'll typically ask their ACO coordinator and follow up. Give them a week before you follow up yourself.
  • Your doctor's ACO isn't enrolled and has no plans to join. In this case, they may still be willing to discuss CBD as a wellness option and provide guidance on dosing and drug interactions even if the free benefit isn't available to you.
  • Your doctor is skeptical of CBD. This happens. Stay factual. You can note that the BEI is a CMS-approved program, not a fringe idea. If they remain resistant, that's their prerogative, and you can explore purchasing CBD directly.

What to Mention About Your Health

Framing CBD in terms of specific symptoms helps your doctor take it more seriously. Instead of just saying "I want to try CBD," try:

  • "I've been having trouble sleeping through the night and want to explore non-pharmaceutical options."
  • "I have chronic knee pain that I'd like to manage without increasing my pain medication."
  • "I've heard CBD can help with inflammation and I wanted your take on whether it might help my arthritis."

These are legitimate clinical conversations. CBD dosing and interactions are real medical considerations, and your doctor can actually help you do this safely.

What to Ask About Drug Interactions

If you take any regular medications, ask your doctor specifically: "Could CBD interact with any of my current medications?" The most important ones to flag are blood thinners (warfarin, eliquis), some heart medications, and anticonvulsants. CBD uses the same liver enzymes as many common drugs, so your doctor needs to know about it.

Bring a full list of your medications to the appointment if you don't already have one handy.

What If You're Nervous About Asking?

Write it down. Literally bring a printed note if you need to. Something like: "I'd like to ask about the Substance Access BEI for CBD, and whether it's available to me as a Medicare patient." Handing a note to your doctor or handing it to the nurse to add to your chart takes the verbal pressure off completely.

You could also send a message through your patient portal before the appointment to give your doctor time to look into it before you meet.
