Is CBD Covered by Insurance?

CBD is not covered by standard private health insurance. And standard Medicare and Medicaid don't cover it either. But there is one real pathway for eligible patients to receive CBD at no cost: the Substance Access Benefit Enhancement Initiative (BEI), a CMS pilot program that lets certain ACOs provide qualifying CBD to their Medicare patients for free.

Why Doesn't Private Insurance Cover CBD?

Insurance companies cover medications and treatments that are FDA-approved, clinically established, and have billing codes. CBD, for general wellness use, doesn't fit that mold yet. The FDA hasn't approved it as a treatment for any common condition except two rare forms of epilepsy (via a drug called Epidiolex). Without FDA approval and a diagnostic code, private insurers have no mechanism to cover it.

That's not a judgment on whether CBD works. It's a structural issue with how insurance coverage gets determined in the U.S.

What About Health Sharing Plans or Supplemental Insurance?

Health sharing ministries and supplemental insurance plans (like those for vision or dental) operate under different rules than traditional insurance, but they also don't cover CBD. They typically only cover items that qualify under IRS rules for medical expenses, and CBD doesn't qualify for that either unless it's been prescribed by a doctor for a specific medical condition.

Is There Any Insurance That Covers CBD?

Not in the traditional sense. The closest thing to insurance coverage for CBD is the BEI program. Technically it's not insurance at all. It's a CMS-funded supplemental benefit delivered through ACOs. But the practical effect is the same: eligible patients receive qualifying CBD products at no personal cost.

Some Medicare Advantage plans have also started including hemp wellness products in their OTC (over-the-counter) supplemental benefit cards. This varies by plan, region, and year. What About Medicaid?

Standard Medicaid does not cover CBD. Some states have Medicaid waiver programs with expanded benefits, but none currently include hemp CBD supplements as a covered benefit.

What Is the BEI and How Is It Different?

The Substance Access BEI is a pilot program within the CMS ACO REACH model. It's not standard Medicare, and it's not private insurance. It's a structured supplemental benefit that certain ACOs have opted into. Through it, eligible patients receive up to $500 per year in qualifying CBD products.

This program is the result of CMS wanting to expand non-pharmaceutical wellness options for Medicare patients, particularly for managing chronic pain, sleep difficulties, and inflammation. The ACO model lets this happen with clinical oversight.

Could Insurance Coverage Expand in the Future?

It's possible. The BEI is a pilot, and pilots that show positive outcomes can evolve into permanent programs. If the BEI demonstrates measurable improvements in patient outcomes or cost reductions, CMS could expand it. And if FDA ever approves CBD products for more common conditions, private insurance coverage could follow.

For now, though, the BEI is the main channel, and it's only for Medicare patients with an enrolled ACO.

What Are My Options If Insurance Doesn't Cover My CBD?

Several practical options exist:

  1. Ask about the BEI: Ask your primary care doctor if their ACO participates
  2. Check your Medicare Advantage supplemental benefits: Your OTC benefit card may cover hemp products
  3. Buy direct from the farm: Edify sells CBD at farm-direct prices, cutting out retail markup
  4. Subscribe and save: Our subscription program lowers costs further

How Much Does It Cost Out of Pocket?

This is a valid concern with a crowded CBD market. Many products are mislabeled or untested. Edify publishes a Certificate of Analysis for every batch, accessible via QR code on the package. These are produced by independent, accredited labs and verify potency, purity, and contaminant screening.
