Does Medicare Advantage Cover CBD?

Medicare Advantage can cover CBD through two separate channels: the Substance Access Benefit Enhancement Initiative (BEI) and optional supplemental benefits that some Medicare Advantage plans offer on their own. If your doctor's ACO is enrolled in the BEI, you may qualify regardless of which Medicare plan you have. Additionally, some Medicare Advantage plans include wellness benefits that cover products like hemp supplements.

What Is Medicare Advantage?

Medicare Advantage (Part C) is an alternative way to receive your Medicare benefits. Instead of getting coverage directly from the federal government, you enroll in a private insurance plan that has a contract with Medicare. These plans must cover everything Original Medicare covers, but they can also offer extra benefits that Original Medicare doesn't.

Those extra benefits are called supplemental benefits. They've expanded significantly in recent years to include things like vision, dental, hearing, transportation, and in some cases, wellness products including CBD or hemp supplements.

The BEI and Medicare Advantage: How They Interact

The BEI is a program that operates at the ACO level, not the plan level. An ACO is a group of doctors and healthcare providers that coordinate care for Medicare patients. An ACO can include patients who are in Original Medicare and patients who are in Medicare Advantage plans.

If your doctor's ACO participates in the BEI, you can access the CBD benefit through that channel regardless of whether you're on Original Medicare or a Medicare Advantage plan. Your insurance card type doesn't determine BEI eligibility. Your doctor's ACO affiliation does.

What About My Medicare Advantage Plan's Own Wellness Benefits?

This is separate from the BEI. Some Medicare Advantage plans have added hemp or CBD supplements to their supplemental benefit offerings. Not all plans do this, and it varies widely by insurer and region.

To find out if your plan includes CBD or hemp wellness products:

  1. Look at your plan's Evidence of Coverage or Summary of Benefits
  2. Search for terms like "wellness," "OTC benefit," or "supplemental health products"
  3. Call your plan's member services line and ask directly: "Does my plan cover CBD or hemp wellness supplements?"

Some plans offer an OTC (over-the-counter) benefit card that can be used for a range of approved wellness products, and some administrators have added qualifying CBD to that list.

Can I Access Both the BEI and My Plan's Supplemental Benefit?

Potentially, yes. The BEI and your Medicare Advantage plan's supplemental benefits are separate programs. If you qualify for the BEI through your doctor's ACO AND your plan offers its own OTC wellness benefit that includes CBD, you may be able to access both. Check with your ACO coordinator and your plan's member services to understand the specifics.

What Products Might Medicare Advantage Cover?

Through the BEI pathway, qualifying products must meet CMS standards: hemp-derived, accurately labeled, third-party tested, with no prohibited additives. How Are Medicare Advantage Plans Expanding Wellness Coverage?

CMS has given Medicare Advantage plans more flexibility to offer supplemental benefits that address social determinants of health and overall wellness, not just medical treatments. This flexibility is called SSBCI (Special Supplemental Benefits for the Chronically Ill) and it's enabled some plans to cover products like CBD for patients with chronic conditions.

If you have a chronic condition being managed by your Medicare Advantage plan, ask your plan if you qualify for any SSBCI benefits that include hemp wellness products.

Is My Doctor Aware of These Options?

Possibly not. Many primary care physicians haven't heard of the BEI or the specific supplemental benefits their patients' Medicare Advantage plans offer. Bringing this up yourself is often the most effective first step.
