The $500 Medicare CBD Benefit Explained

The $500 Medicare CBD benefit comes from a CMS Innovation Center program called the Substance Access Benefit Enhancement Initiative (BEI). Under this program, Accountable Care Organizations (ACOs) that participate can provide up to $500 per year in qualifying CBD products to eligible Medicare patients at no charge. It's a real benefit, not a rumor, and it's specifically designed to give seniors access to hemp-based wellness products through their healthcare team.

Quick Facts
  • The $500 cap is per patient, per year
  • It's funded through ACO participation in the CMS Substance Access BEI
  • Patients pay nothing out of pocket for qualifying products
  • The benefit resets annually
  • ACOs choose which products to include within CMS guidelines

Where Does the $500 Come From?

CMS (the Centers for Medicare and Medicaid Services) funds the BEI as part of its broader ACO REACH model. ACOs that join this program receive enhanced support for coordinated care, and in return they take on more accountability for patient outcomes. The $500 per patient for CBD is part of the supplemental benefits ACOs can offer to support whole-person wellness.

The money isn't sent directly to patients as cash. It's allocated by the ACO to purchase qualifying products on the patient's behalf. Think of it like a wellness credit that only applies to approved products.

How Is the $500 Used?

Each ACO manages its own allocation. Some might give patients one full-spectrum CBD oil per month. Others might offer a mix of gummies and oils. The $500 annual cap covers the cost of whatever the ACO provides.

Here's a rough example of what $500 in CBD could look like:

  • 10 to 12 bottles of full-spectrum CBD oil over the year
  • A monthly supply of CBD gummies for 8 to 10 months
  • A combination of oils and gummies distributed as needed

The actual product selection depends entirely on what your ACO has approved. Edify's Full Spectrum CBD Oil with CBG and CBD Gummies for Pain are examples of products designed to meet BEI quality requirements.

Does the $500 Reset Every Year?

Yes. The $500 benefit is an annual cap that resets each calendar year. If your ACO only uses $300 of your allocation one year, the remaining $200 doesn't carry over. The cap starts fresh each year for eligible patients.

It's worth asking your ACO coordinator at the start of each new year what's available and how to use your full allocation.

Can I Get the $500 in Cash Instead?

No. The benefit doesn't work as a cash payment. The ACO uses the allocation to purchase qualifying CBD products and provides them to you directly. You can't ask to receive the cash equivalent or use it for other products.

What Products Does the $500 Cover?

The BEI covers hemp-derived CBD products that meet CMS standards. These include:

  • Full-spectrum CBD oils
  • Broad-spectrum (THC-free) CBD oils
  • CBD gummies in various formulas
  • CBD capsules and other approved forms

Who Can Access the $500 Benefit?

To access this benefit, you need to:

  1. Be enrolled in Medicare (Original Medicare or a Medicare Advantage plan)
  2. Have a primary care provider whose ACO participates in the Substance Access BEI
  3. Meet any eligibility criteria your specific ACO has set

The biggest barrier for most patients is that their doctor's ACO isn't enrolled yet. If that's your situation, see our section below on what to do.

Check out the full eligibility breakdown on our Medicare CBD eligibility page.

What If My ACO Isn't Enrolled?

You have real options. First, you can ask your doctor if they're aware of the BEI and whether their ACO plans to join. Many physicians haven't heard of it yet, and patients asking questions can move this forward. Second, you can ask if there's a participating provider in your area you could be referred to. Third, you can purchase CBD directly from Edify at farm-direct prices.

Edify sells directly from our Kentucky family farm. No retail markup, no middleman. Our subscribe and save option makes it even more affordable month to month.

How Does This Fit With the Rest of Medicare?

The $500 BEI benefit is separate from your standard Medicare benefits. It doesn't affect your Part A or Part B coverage. It doesn't count toward your deductible or out-of-pocket maximum. It's an entirely separate supplemental benefit administered by your ACO.

If you have a Medicare Advantage plan, you may also have supplemental wellness benefits through your plan. These are different from the BEI. It's worth checking both to see what you're entitled to. Learn more on our Medicare Advantage CBD coverage page.

Is $500 Enough?

For many seniors, yes. A quality CBD oil might cost $40 to $60 per bottle. A monthly supply of CBD gummies might run $35 to $50. The $500 annual cap covers a meaningful amount of product. It's enough to try CBD consistently for several months and see how it works for you.

If you end up wanting more than the $500 covers, Edify's farm-direct pricing makes it easy to supplement your BEI allocation without breaking your budget.

Why Did CMS Create This Benefit?

CMS Innovation Center programs are designed to test new approaches to healthcare delivery. The Substance Access BEI is an experiment in giving ACOs more tools to support patient wellness with non-pharmaceutical options. If the program shows positive outcomes, it could become a more permanent part of Medicare's supplemental benefit framework.

It reflects a broader shift in how Medicare thinks about wellness. Pain management, sleep support, and inflammation reduction are areas where CBD has shown real promise in patient-reported outcomes, even without the full FDA approval process.

How Do I Know the Products Are Safe?

Because qualifying products must be third-party tested, you should always be able to see lab results. At Edify, every product has a QR code that links directly to the Certificate of Analysis for that specific batch. No guessing, no trust-me labels.

We're also USDA organic, which means our hemp is grown without synthetic pesticides or fertilizers. Our USDA organic farm page has more on our growing practices. All results are published on our lab results and COA page.
