The best starting point is innovation.cms.gov, which lists every ACO participating in CMS innovation models by state. You can search for Delaware-based or Delaware-adjacent ACOs from there.

Christiana Care Health System is the largest health network based in Delaware and is part of the LifePoint Health network. It has affiliated practices throughout New Castle County and parts of southern Delaware. Jefferson Health and Penn Medicine, both based in Philadelphia, also serve Delaware patients and operate large ACO programs.

If your doctor is based in the Philadelphia or Baltimore metro and you're a Delaware resident, you may already be connected to an ACO. Ask your primary care provider: are you part of an ACO, and is that ACO enrolled in the Substance Use Access BEI? If they're unsure, ask the office administrator or your Medicare Advantage plan's member services team.

Delaware Medicaid and Medicare Dual Eligibility

Delaware's Medicaid program is called Diamond State Health Plan. Seniors who qualify for both Medicare and Medicaid are considered dually eligible and often have access to Dual Eligible Special Needs Plans (D-SNPs).
