Christiana Care Health System is the largest health network based in Delaware and is part of the LifePoint Health network. It has affiliated practices throughout New Castle County and parts of southern Delaware. Jefferson Health and Penn Medicine, both based in Philadelphia, also serve Delaware patients and operate large ACO programs.
If your doctor is based in the Philadelphia or Baltimore metro and you're a Delaware resident, you may already be connected to an ACO. Ask your primary care provider: are you part of an ACO, and is that ACO enrolled in the Substance Use Access BEI?
