How to Get CBD From Your Doctor Through Medicare
Your doctor can provide qualifying CBD products at no cost to you through a CMS program called the Substance Access Benefit Enhancement Initiative (BEI), if their Accountable Care Organization (ACO) is enrolled. The process is straightforward: you ask, they confirm eligibility, and the ACO handles the rest. Here's exactly how to make it happen.
For background on how this benefit works, see our Medicare CBD overview.
Quick Facts- CBD is available at no cost through Medicare if your doctor's ACO participates in the BEI
- You initiate the process by asking your primary care doctor
- No prescription is needed, but your doctor must enroll you in the benefit
- Products are delivered through the ACO, not picked up at a pharmacy
- Eligible patients can receive up to $500 per year in qualifying products
Step 1: Ask Your Primary Care Doctor
Start at your next appointment. You don't need to prepare a formal request. Just bring it up naturally. Here's a phrase that works well:
"I've been reading about a Medicare program that lets some ACOs provide CBD products to patients at no cost. It's called the Substance Access BEI. Does our practice participate?"
Most doctors will either know the answer immediately or take a moment to check with their office administrator. If they're not sure, give them a day to find out.
For a more detailed conversation guide, see our page on how to talk to your doctor about CBD and Medicare.
Step 2: Confirm Your Eligibility
If your doctor's ACO does participate, the next step is confirming your eligibility. Most ACOs require that you:
- Are enrolled in Medicare (Original or Advantage)
- Are an active patient in their ACO
- Meet any clinical criteria the ACO has set (such as managing a chronic condition)
Check the full eligibility requirements on our Medicare CBD eligibility page.
Step 3: Complete Any Required Paperwork
Some ACOs require a brief intake form or a care plan note before adding the CBD benefit to your account. This might be a single page asking about your health goals, current medications, and any known sensitivities. It typically takes just a few minutes.
You may also be asked to participate in a brief wellness survey before and after using the products. This helps the ACO track outcomes and report back to CMS. Participating is generally voluntary, but it supports the program's long-term survival.
Step 4: Choose Your Products
Your ACO will have a list of approved products. You may have some choice within that list, or they may have a set selection. Common options include:
- Full-spectrum CBD oil
- THC-free (broad-spectrum) CBD oil
- CBD gummies for sleep or pain
- CBD capsules
Edify's products are specifically designed to meet BEI quality requirements. If your ACO gives you the option to request specific brands, our CBD Gummies for Sleep, CBD Gummies for Pain, and Full Spectrum CBD Oil with CBG are excellent options to ask about.
Step 5: Receive Your Products
Once enrolled, the ACO manages the ordering and delivery. In most cases, products ship directly to your home. There's no copay, no billing to secondary insurance, and no trip to a pharmacy. You just receive them.
Some ACOs may set a monthly cadence (for example, one bottle of oil or one pouch of gummies per month). Others may distribute quarterly or based on your expressed needs. Ask your care coordinator what the delivery schedule looks like.
Step 6: Track What You've Used
The $500 annual cap resets each year. Keep a rough note of what you've received so you can ask your care coordinator how much of your allocation you've used. If you're approaching the cap and still have months left in the year, ask if there's any way to adjust the distribution timing.
Learn more about how the cap works on our $500 Medicare CBD benefit page.
What If My Doctor's ACO Doesn't Participate?
Don't give up. You have a few good options:
- Ask when they might enroll: Many ACOs are still evaluating the BEI. Your question might accelerate the conversation.
- Ask for a referral: If another provider in your area participates, ask if a referral makes sense.
- Buy direct from Edify: Our farm-direct pricing is significantly lower than retail. Use our subscribe and save option for steady supply and ongoing savings.
What If My Doctor Doesn't Know What the BEI Is?
That's very common. The BEI is a newer program and not every physician has been briefed on it. If your doctor draws a blank, you can suggest they check with their ACO coordinator or look up the CMS ACO REACH Substance Access BEI documentation. You're not asking your doctor to do anything unusual. You're just pointing them toward a program they may not have discovered yet.
Does Getting CBD From My Doctor Affect My Other Medicare Coverage?
No. The BEI is a supplemental benefit layered on top of your standard Medicare coverage. It doesn't affect your Part A, B, or D benefits. It doesn't change your deductible or out-of-pocket maximum. It's additive, not a replacement for anything you already have.
Is the CBD Actually Good Quality?
It should be, because CMS requires third-party testing. But quality can vary between ACOs depending on which vendors they've selected. Edify products meet every BEI quality standard. We're USDA organic, third-party tested on every batch, and our nano-enhanced formula provides superior bioavailability compared to standard oil-based CBD.
